These are confidential, internal reports to be completed by a Ä¢¹½´«Ã½ employee. Their contents are not to be shared or copied for any individual who is not a College employee or a Ä¢¹½´«Ã½ legal representative.
In case of a serious injury, immediately make a telephone report to Public Safety.
